Summary

Severe primary hyperparathyroidism (PHP) significantly increases the risk of metabolic syndrome and insulin resistance. Serum calcium levels are a key predictor of these cardiovascular risk factors in PHP patients.

Background:

Severe primary hyperparathyroidism (PHP) is linked to cardiovascular issues.
The cardiovascular impact of mild PHP remains unclear.
This study investigates cardiovascular risk factors in mild and severe PHP.

Purpose of the Study:

To assess the correlation between mild and severe primary hyperparathyroidism (PHP) and emergent cardiovascular risk factors.
To determine if serum calcium predicts metabolic syndrome and insulin resistance in PHP patients.

Main Methods:

Cross-sectional study of 139 PHP patients (72 severe, 67 mild) and 111 controls.
Measured fasting blood levels of calcium, PTH, lipids, glucose, insulin, and inflammatory markers.
Evaluated BMI, waist circumference, blood pressure, insulin resistance (IR), and metabolic syndrome (MS).

Main Results:

Severe PHP patients exhibited higher rates of MS (37.5%) and IR (38.9%) compared to mild PHP (34.3%, 23.9%) and controls (14.4%, 14.4%).
Severe PHP was significantly associated with increased likelihood of MS (OR 3.5) and IR (OR 3.7).
Serum calcium levels significantly predicted the presence of MS (OR 1.875) and IR (OR 2.043).

Conclusions:

Patients with severe primary hyperparathyroidism (PHP) face a greater probability of developing metabolic syndrome and insulin resistance.
Serum calcium is a significant predictor of these cardiovascular risk factors in PHP.
These findings highlight the importance of monitoring cardiovascular health in PHP patients.
