Rapidly dividing tumors, embryos, and wounded tissues require more oxygen than usual, lowering the oxygen concentration in the blood. At low oxygen or hypoxic conditions, an oxygen-sensitive transcription factor called the hypoxia-inducible factor 1 or HIF1 is activated. Blood vessel formation starts early during embryonic development, around day 7. In the extraembryonic yolk sac, mesodermal precursor cells called hemangioblast proliferate and differentiate into angioblast. Angioblasts express vascular endothelial growth factor receptor 2 or VEGFR2, which binds VEGF-A, a proangiogenic factor, guiding blood vessel formation.     Endothelial progenitor cells (EPCs) are crucial for blood vessel repair. This study found lower EPC levels in patients with ischemic heart disease, correlating with impaired endothelial function.

    Background:

    • Endothelial progenitor cells (EPCs) are vital for reendothelialization and neovascularization.
    • Endothelial dysfunction is a hallmark of ischemic heart disease (IHD).
    • Understanding EPCs' role in IHD pathogenesis is crucial for therapeutic strategies.

    Purpose of the Study:

    • To investigate CD34+ cell counts in patients with different forms of ischemic heart disease (IHD).
    • To assess the association between CD34+ cell numbers and endothelial dysfunction.
    • To explore the role of CD34+ cells in the context of acute coronary syndromes (ACS) and stable angina (SA).

    Main Methods:

    • Quantification of CD34+ cells using flow cytometry.
    • Measurement of brachial artery flow-mediated vasodilatation (FMD) to assess endothelial function.
    • Assessment of common carotid artery intima-media thickness (IMT) via ultrasonography.

    Main Results:

    • CD34+ cell levels were significantly lower in IHD patients compared to controls.
    • The lowest CD34+ cell counts were observed in patients with ST-segment elevation acute coronary syndromes (ACS).
    • Reduced CD34+ cell numbers were strongly associated with endothelial dysfunction (impaired FMD and increased IMT).

    Conclusions:

    • CD34+ cells play a role in repairing injured endothelium.
    • Diminished peripheral blood CD34+ cell counts may serve as an indicator of endothelial health in IHD.
    • Further research into EPC-based therapies for IHD is warranted.
