    Background:

    • Prostate carcinoma can present with unusual paraneoplastic syndromes.

    Observation:

    • A patient with locally recurrent, poorly differentiated prostate carcinoma developed hypokalemic alkalosis, hypernatremia, and hyperosmolar syndrome.
    • Clinical presentation suggested ectopic ACTH syndrome due to elevated plasma ACTH and cortisol, unresponsive to dexamethasone suppression.

    Findings:

    • Autopsy revealed extensive prostate carcinoma with pelvic metastases and adrenal hyperplasia.
    • Prostatic tumor tissue showed significantly elevated ACTH levels and ACTH-positive secretory granules in tumor cells.
