Background:

Endodermal cysts (EC) are rare central nervous system lesions, predominantly spinal.
Intracranial EC are uncommon, with supratentorial locations being exceptional.

Observation:

A 62-year-old male presented with altered mental status after head trauma.
CT revealed a large frontal cyst with significant midline shift.
Cyst fluid was milky with high protein; MRI confirmed the lesion.

Findings:

Complete surgical resection of the frontal EC was achieved.
Pathological examination confirmed a typical endodermal cyst.
The patient recovered fully with no recurrence on follow-up MRI.

Implications:

Supratentorial endodermal cysts are exceptionally rare but require consideration.
Differential diagnosis can be challenging based on imaging alone.
Radical surgical excision is the recommended treatment to prevent recurrence.
