  • Intracranial EC are uncommon, with supratentorial locations being exceptional.

  • A 62-year-old male presented with altered mental status after head trauma.
  • CT revealed a large frontal cyst with significant midline shift.
  • Cyst fluid was milky with high protein; MRI confirmed the lesion.

Findings:

  • Complete surgical resection of the frontal EC was achieved.
  • Pathological examination confirmed a typical endodermal cyst.
  • The patient recovered fully with no recurrence on follow-up MRI.

Implications:

  • Supratentorial endodermal cysts are exceptionally rare but require consideration.
  • Differential diagnosis can be challenging based on imaging alone.
  • Radical surgical excision is the recommended treatment to prevent recurrence.
