Summary

Microvascular decompression failed to relieve trigeminal neuralgia in a patient. Choroid plexus compressing the trigeminal nerve root entry zone was identified as a potential cause.

Background:

  • A 55-year-old male with pharmacoresistant trigeminal neuralgia (TN) underwent microvascular decompression.
  • Preoperative MRI indicated a neurovascular conflict involving the trigeminal nerve root and the superior cerebellar artery.

Observation:

  • Surgical exploration revealed choroid plexus compressing the trigeminal nerve root entry zone.
  • Partial removal of the choroid plexus was performed.

Findings:

  • The patient experienced no pain relief following microvascular decompression.
  • Trigeminal neuralgia may result from mechanical irritation of the intracisternal trigeminal nerve root.

Implications:

  • Vascular decompression alone may be insufficient for TN when other pathologies are present.
