Background:

  • Occupational rhinitis (OR) is a common condition affecting workers.
  • Accurate diagnosis of OR can be challenging.
  • Specific nasal provocation tests (sNPT) are used, but their interpretation can be improved.

Purpose of the Study:

  • To determine the utility of analyzing nasal blown secretions during sNPT for diagnosing occupational rhinitis.
  • To establish eosinophil levels in nasal secretions as a diagnostic marker for OR.

Main Methods:

  • Nasal secretions were collected from healthy subjects, allergic rhinitis patients, and individuals with work-related rhinitis symptoms (WRRS).
  • Secretions were analyzed before and after sNPT.
  • sNPT positivity was determined by rhinoscopy and nasal symptom scores.

Main Results:

  • A high percentage (89.6%) of WRRS subjects yielded suitable nasal secretions.
  • Eosinophil counts significantly increased in nasal secretions following positive sNPTs.
  • A cut-off of 4% and/or 1 x 10(4) eosinophils/ml indicated a significant post-sNPT increase.
  • 33% of WRRS subjects with initially negative sNPTs showed significant eosinophil increases, suggesting possible OR.

Conclusions:

  • Eosinophil evaluation in nasal blown secretions is a valuable tool for monitoring responses to occupational nasal challenges.
  • This method enhances the diagnostic accuracy of occupational rhinitis.
