This study introduces a novel transperitoneal laparoscopic partial nephrectomy technique for upper pole kidney tumors. The upside-down kidney approach offers improved visualization and acceptable operative times with manageable complications.

Area of Science:

  • Urology
  • Minimally Invasive Surgery
  • Nephrology

Background:

  • Transperitoneal laparoscopic partial nephrectomy is a standard procedure for kidney tumors.
  • Managing upper pole renal lesions can present unique surgical challenges.
  • A novel intraoperative kidney rotation technique was developed to address these challenges.

Purpose of the Study:

  • To evaluate the feasibility and outcomes of a new transperitoneal laparoscopic partial nephrectomy technique.
  • To assess the efficacy of intraoperative kidney rotation for upper pole renal masses.
  • To analyze operative time, complications, and oncological results of this innovative approach.

Main Methods:

  • Ten patients with upper pole renal lesions underwent transperitoneal laparoscopic partial nephrectomy.

  • The kidney was mobilized and rotated 180 degrees to position the upper pole inferiorly.
  • Resection beds were closed with sutures and clips, followed by kidney repositioning.
  • Main Results:

    • The median tumor size was 58 mm; 9 patients had contrast-enhancing masses, 1 had a nonfunctioning pole.
    • Median operative time was 206 minutes, with a mean warm ischemia time of 30 minutes.
    • Negative surgical margins were achieved in all patients; complications included one instance of bleeding and one of prolonged urine leakage.

    Conclusions:

    • Intraoperative kidney rotation during laparoscopic partial nephrectomy for upper pole lesions is feasible.
    • This technique provides acceptable operative times and complication rates.
    • The upside-down approach enhances surgical field visualization, facilitating the procedure.
