Computed tomography (CT) findings in immunocompromised patients with abdominal aspergillosis often show angioinvasive features or abscesses. Familiarity with these CT findings can aid in diagnosing this rare and fatal condition.

Background:

  • Abdominal aspergillosis is a rare but fatal invasive fungal infection.
  • Immunocompromised patients, particularly those with hematologic malignancies or solid organ transplants, are at high risk.

Observation:

  • This study retrospectively evaluated CT findings in six immunocompromised patients with pathologically confirmed abdominal aspergillosis.
  • CT scans were analyzed for lesion characteristics, including location, number, morphology, attenuation, and contrast enhancement.

Findings:

  • Aspergillosis involved various abdominal organs, including blood vessels, liver, spleen, gastrointestinal tract, kidneys, peritoneum, and retroperitoneum.
  • CT demonstrated angioinvasive features such as vascular thrombosis and pseudoaneurysm leading to organ infarction or perforation.
  • Other findings included low-attenuating lesions resembling abscesses, bowel wall thickening mimicking typhlitis, and peritoneal/retroperitoneal infiltration.

Implications:

  • Recognizing characteristic CT findings of abdominal aspergillosis is crucial for early diagnosis.
  • Familiarity with these imaging features can improve diagnostic accuracy for this life-threatening infection.
