Summary

Intracranial hemangiomas are rare benign tumors that can mimic meningiomas in the cavernous sinus. Surgical removal presents challenges due to the risk of bleeding.

Observation:

  • A 60-year-old woman presented with third cranial nerve palsy.
  • Imaging revealed a giant extraaxial mass in the middle cranial fossa.
  • Angiography showed mild enhancement and mass effect.

Findings:

  • Tumoral embolization was performed.
  • The hemangioma was partially resected via a pterional approach.
  • The patient had an uneventful recovery without new deficits.

Implications:

  • Hemangiomas should be considered in the differential diagnosis of cavernous sinus tumors.
