This case report details a rare congenital auricular arteriovenous malformation (AVM) in an elderly man. Surgical excision with skin grafting was performed due to complications like ulceration and hemorrhage.

Background:
- Arteriovenous malformations (AVMs) are uncommon vascular anomalies, typically intracranial but can occur in the head and neck.
- Congenital AVMs may manifest later in life, exacerbated by factors like puberty or pregnancy.
Observation:
- A rare, disfiguring congenital auricular AVM was observed in a 77-year-old male.
- The lesion presented as a large, pulsatile, ulcerative, and hemorrhagic swelling of the pinna.
- Angiography confirmed the AVM was supplied by superficial temporal and occipital arteries.
Findings:
- Complete auricular excision with split-skin grafting was the chosen treatment.
- Preoperative embolization was contraindicated due to the patient's renal dysfunction and the high-flow nature of the AVM.
Implications:
- This case highlights the importance of considering AVMs in complex head and neck presentations.
- Surgical management, even in elderly patients with comorbidities, can be successful for auricular AVMs.
- The challenges of managing high-flow AVMs in patients with renal impairment are demonstrated.
