This study introduces a modified laparoscopic Y-V pyeloplasty technique to treat ureteropelvic junction (UPJ) obstruction in patients with enlarged renal pelvis. The new method successfully reduced renal pelvis size and eliminated obstruction in all patients.
Area of Science:
- Urology
- Minimally Invasive Surgery
Background:
- Y-V pyeloplasty is a standard procedure for ureteropelvic junction (UPJ) stenosis.
- Traditional Y-V pyeloplasty is not recommended for patients with enlarged renal pelvis due to lack of pelvic size reduction.
Purpose of the Study:
- To present a novel laparoscopic Y-V pyeloplasty technique.
- To address UPJ stenosis in patients with enlarged renal pelvis.
- To enable simultaneous reduction of renal pelvis size.
Main Methods:
- A modified Y-V pyeloplasty was performed.
- The cranial limbs of the Y-shaped incision were extended elliptically.
- This modification aimed to eliminate redundant pelvic tissue and reduce pelvic volume.
Main Results:
- Three female patients (ages 20-60) with UPJ stenosis and enlarged renal pelvis underwent the procedure.
- All patients experienced an uneventful postoperative course.
- Follow-up (10-43 months) showed no evidence of obstruction.
Conclusions:
- The modified laparoscopic Y-V pyeloplasty is effective for UPJ stenosis with enlarged renal pelvis.
- This technique expands the applicability of Y-V pyeloplasty.
- It offers a solution for patients previously excluded from this procedure.
