Background:

Stump appendicitis is a rare, delayed complication following an appendectomy.
Few cases have been documented in pediatric literature.

Purpose of the Study:

To report a case of stump appendicitis in a child.
To highlight the diagnostic challenges of this condition.

Main Methods:

A 9-year-old boy presented with right lower quadrant pain, vomiting, and fever post-appendectomy.
Imaging revealed a fecalith; diagnosis was confirmed intraoperatively.
Surgical management involved complete removal of the appendiceal stump.

Main Results:

The patient had a gangrenous and perforated appendiceal stump.
Post-operative recovery was uneventful over 18 months.

Conclusions:

Stump appendicitis should be suspected in patients with right lower quadrant pain, even with prior appendectomy.
Complete appendiceal stump excision is essential for prevention.
