Summary

A rare case of an intraconal abscess, originating from a sinusitis-affected Onodi cell, was successfully treated. This orbital abscess in a healthy patient resolved completely with intravenous antibiotics.

Background:

  • Orbital abscesses are uncommon, particularly those secondary to paranasal sinusitis.
  • Onodi cells, posterior ethmoid air cells extending superior to the sphenoid sinus, are rarely implicated in orbital infections.

  • A 63-year-old female presented with decreased vision, limited eye movement, and optic neuropathy without overt ocular inflammation.
  • Magnetic resonance imaging revealed a well-encapsulated intraconal mass and an Onodi cell.
  • Surgical exploration identified an abscess containing Staphylococcus aureus.

Findings:

  • Successful surgical drainage and intravenous antibiotic therapy led to complete resolution of the orbital abscess and associated symptoms.
  • The case highlights the importance of considering sinusitis as a potential source of deep orbital infections.
