Purpose of the Study:

  • To evaluate the association between parathyroid hormone (PTH) levels and cognitive decline in an aging population.
  • To assess the predictive value of PTH for cognitive decline over a 10-year period.

Main Methods:

  • A longitudinal prospective study of 514 individuals aged 75, 80, and 85 years.
  • Cognitive function assessed using Mini-Mental State Examination (MMSE) and Clinical Dementia Rating (CDR) at baseline and over 10 years.
  • Baseline measurements included serum PTH, ionized calcium (Ca2+), creatinine, and apolipoprotein E (APOE) alleles.

Main Results:

  • Elevated PTH (> or =62 ng/L) at baseline was associated with impaired cognition (MMSE<24 or CDR> or =1).
  • High PTH indicated a 2-fold risk of MMSE decrease and 3-fold risk of CDR increase within one year, independent of confounders.
  • PTH predicted cognitive decline within five years, but this association diminished by ten years.

Conclusions:

  • Elevated PTH is associated with cognitive decline over a five-year period in the general aged population.
  • This association is independent of calcium and renal function.
  • The role of vitamin D deficiency in elevated PTH and subsequent cognitive decline warrants further research.
