Background:

Papillary fibroelastomas (PFEs) are the most common primary cardiac tumors, typically benign and solitary, often found on heart valves.
PFEs usually occur in older men (mean age 56) and are characterized by a sea anemone-like appearance with frond-like projections.

Observation:

A unique case of a 25-year-old man presenting with a massive left middle cerebral artery stroke and hemiplegia.
Investigations revealed a left ventricular outflow tract lesion, later confirmed as a multifocal, carpet-like papillary fibroelastoma.

Findings:

Histopathological examination revealed multiple PFEs with a characteristic tree-trunk-like structure and central elastic tissue.
This represents the first reported case of surgically resected and morphologically confirmed multiple PFEs removed in a single procedure.

Implications:

This case expands the known clinical presentations of papillary fibroelastomas, particularly in younger individuals.
The successful surgical removal of multifocal PFEs underscores the importance of thorough investigation and intervention in suspected cardiac tumors presenting with neurological deficits.
