Neurocognitive symptoms in primary hyperparathyroidism predict parathyroid hyperplasia. Three or more symptoms combined with a negative sestamibi scan accurately identified hyperplasia in patients.

Background:

  • Primary hyperparathyroidism (PHP) is a common endocrine disorder.
  • Parathyroid hyperplasia is a key pathological finding in PHP.

Purpose of the Study:

  • To investigate the association between symptomatic presentation and parathyroid hyperplasia.
  • To determine if preoperative symptoms predict hyperplasia in PHP patients.

Main Methods:

  • Retrospective analysis of 111 PHP patients undergoing parathyroidectomy.
  • Comparison of preoperative symptom profiles between patients with and without hyperplasia.

Main Results:

  • Neurocognitive symptoms were present in 51.4% of patients.
  • Increased neurocognitive symptoms linearly correlated with higher risk of hyperplasia (3+ symptoms: 61%).
  • A negative sestamibi scan combined with 3+ neurocognitive symptoms predicted 100% hyperplasia.

Conclusions:

  • Neurocognitive dysfunction serves as a predictor for hyperplastic parathyroid disease in PHP.
  • A specific symptom cluster (3+ neurocognitive symptoms and negative sestamibi scan) offers definitive prediction of hyperplasia.
