Summary

This case report details a rare instance of granulomatous hypophysitis affecting the pituitary and sphenoid sinus in a postpartum woman. The condition, presenting with headaches and vision loss, surprisingly resolved spontaneously within five months.

Background:

  • Granulomatous hypophysitis is a rare inflammatory condition affecting the pituitary gland.
  • Sphenoid sinus involvement is uncommon, and postpartum presentation is unusual.

Observation:

  • A 29-year-old woman presented with headaches and visual field deficits 11 days postpartum.
  • MRI revealed a sellar mass with suprasellar extension and sphenoid sinus mucosal thickening.
  • Histopathology confirmed nonvasculitic granulomatous tissue in the pituitary and sphenoid sinus.

Findings:

  • The patient underwent subtotal resection via a transsphenoidal approach.
  • Hormonal assays indicated a need for desmopressin and levothyroxine replacement.
  • Follow-up imaging at 5 months showed spontaneous resolution of the residual mass.

Implications:

  • This case highlights the unique presentation of granulomatous hypophysitis in the early postpartum period.
  • Concomitant pituitary and sphenoid sinus involvement with spontaneous resolution is rare.
  • The findings contribute to understanding the varied clinical course of granulomatous hypophysitis.
