Background:

Observation:

  • Two cases of enalapril-induced angioedema are presented.
  • The patients experienced oropharyngeal swelling as a side effect of enalapril therapy.

Findings:

  • The mechanism is likely not a direct allergic reaction.
  • Enalapril inhibits ACE, leading to the accumulation of bradykinin and substance P.
  • Accumulated bradykinin and substance P may trigger angioedema.

Implications:

  • Physicians should consider ACE inhibitors as a potential cause of angioedema.
  • Prompt recognition and management of enalapril-induced angioedema are crucial.
  • Understanding the pathophysiology aids in preventing and treating this adverse effect.
