Oral fingolimod or intramuscular interferon for relapsing multiple sclerosis

Fingolimod significantly reduced relapse rates and improved MRI outcomes in multiple sclerosis patients compared to interferon beta-1a. Further studies are needed to evaluate long-term safety and efficacy.

Background:
- Fingolimod (FTY720) is a sphingosine-1-phosphate receptor modulator.

Purpose of the Study:

Main Methods:
- A 12-month, double-blind, double-dummy study.
- 1292 patients with relapsing-remitting MS were randomized to receive oral fingolimod (1.25 mg or 0.5 mg daily) or intramuscular interferon beta-1a (30 microg weekly).
- Primary endpoint: annualized relapse rate. Secondary endpoints: MRI lesion changes and sustained disability progression.
Main Results:
- Fingolimod significantly reduced annualized relapse rates in both dosage groups compared to interferon beta-1a (P<0.001).
- MRI findings corroborated the primary results, showing improved outcomes with fingolimod.
- No significant difference in sustained disability progression was observed between groups.
- Fatal infections (disseminated varicella zoster, herpes simplex encephalitis) occurred in the 1.25 mg fingolimod group. Other adverse events included nonfatal infections, bradycardia, hypertension, macular edema, skin cancer, and elevated liver enzymes.
Conclusions:
- Oral fingolimod demonstrated superior efficacy over intramuscular interferon beta-1a in reducing relapse rates and improving MRI outcomes for MS patients.
- Longer-term studies are necessary to fully assess the safety and efficacy profile of fingolimod beyond one year.
