Disorders of Leukocytes

Leukocyte disorders can lead to either leukopenia, characterized by an abnormally low leukocyte count, or leukocytosis, marked by a very high leukocyte number.
Leukopenia may result from bone marrow disorders, autoimmune diseases, and infectious diseases. For example, conditions such as multiple myeloma and aplastic anemia can impair the bone marrow's ability to produce adequate leukocytes. Summary

Ophthalmic complications in acute lymphoblastic leukemia (ALL) can be fatal. This case highlights the critical need for ongoing eye exams in leukemia patients, even after initial symptoms resolve, due to risks like cytomegalovirus retinitis and Graft-versus-Host Disease.

Background:

  • Acute lymphoblastic leukemia (ALL) can present with diverse ocular manifestations.
  • Ophthalmic findings in leukemia patients require careful monitoring and management.

Observation:

  • A 27-year-old woman with ALL developed Roth's spots and serous retinal detachment.
  • Initial serous retinal detachment resolved, but cytomegalovirus (CMV) retinitis and keratoconjunctivitis due to Graft-versus-Host Disease (GVHD) later occurred.

Findings:

  • Serous retinal detachment in leukemia patients is associated with a poor prognosis.
  • CMV retinitis and GVHD-related ocular complications can arise following initial retinal detachment.

Implications:

  • Periodic ophthalmologic examinations are crucial for leukemia patients with a history of serous retinal detachment, even after symptom resolution.
  • Early detection and intervention for secondary ocular complications like CMV retinitis and GVHD are vital.
