Intravenous immunoglobulin (IVIg) offers a steroid-sparing option for recurrent-relapsing autoimmune optic neuropathy, stabilizing vision in most patients. This treatment reduced long-term corticosteroid dependency in a majority of cases.
Area of Science:
- Neuro-ophthalmology
- Immunology
- Autoimmune diseases
Background:
- Recurrent-relapsing inflammatory optic neuropathies can lead to severe vision loss.
- Long-term corticosteroid use preserves vision but often causes significant side effects.
- Alternative treatments are needed for steroid-dependent cases.
Purpose of the Study:
- To evaluate the efficacy of intravenous immunoglobulins (IVIg) in managing steroid-responsive recurrent-relapsing optic neuropathies.
- To assess IVIg as a steroid-sparing agent in this patient population.
Main Methods:
- A semi-prospective case series involving 6 patients across 4 medical centers.
- Patients had steroid-responsive recurrent-relapsing optic neuropathy.
- Outcomes measured included visual status, remission rates, duration of corticosteroid use, and adverse events.
Main Results:
- Vision stabilized in all 6 patients treated with IVIg, with no improvement or worsening.
- Five out of six patients were able to discontinue corticosteroids after IVIg treatment.
- One patient experienced an adverse event during IVIg infusion; two patients had late relapses on IVIg.
Conclusions:
- Intravenous immunoglobulin (IVIg) is an effective steroid-sparing treatment for selected patients with steroid-dependent recurrent-relapsing autoimmune optic neuropathy.
- IVIg can help maintain stable vision and reduce reliance on long-term corticosteroids.
