Summary

Early avascular filtering blebs after glaucoma surgery predict lower long-term intraocular pressure (IOP). This finding aids in managing primary open-angle glaucoma patients post-trabeculectomy.

Area of Science:

  • Ophthalmology
  • Glaucoma Research
  • Surgical Outcomes

Background:

  • Trabeculectomy is a common surgical procedure for primary open-angle glaucoma.
  • Assessing early postoperative indicators can help predict long-term surgical success.
  • Filtration bleb characteristics are crucial for successful IOP reduction.

Purpose of the Study:

  • To compare early prognostic indicators (first 2 postoperative days) with filtration status 3 months after trabeculectomy.
  • To identify early signs that predict successful IOP control in glaucoma patients.

Main Methods:

  • A cohort of 51 primary open-angle glaucoma patients undergoing trabeculectomy was studied.
  • Prognostic indicators assessed within the first 2 postoperative days included bleb vascularity, height, breadth, transparency, microcysts, and early IOP.

Related Experiment Videos

  • Filtration status and IOP were evaluated 3 months postoperatively.
  • Main Results:

    • Patients with an avascular filtering bleb on postoperative day 2 had significantly lower intraocular pressure (IOP) at 3 months (12.6 mm Hg) compared to those with vascular blebs (16.3 mm Hg).
    • No significant correlation was found between bleb height, breadth, transparency, presence of microcysts, anterior chamber depth, or early postoperative IOP and 3-month filtration status.

    Conclusions:

    • Early avascularity of the filtering bleb is a significant positive prognostic indicator for IOP control 3 months after trabeculectomy.
    • Other early postoperative factors assessed do not reliably predict long-term filtration status in primary open-angle glaucoma patients.
