Summary

Neuro-Behcet's disease (NBD) can cause rare myelitis. This case highlights extensive longitudinal myelitis, responding well to steroids, suggesting NBD as a key differential diagnosis.

Background:

  • Neuro-Behcet's disease (NBD) is a rare neurological complication of Behcet's disease.
  • Myelitis, or inflammation of the spinal cord, is an uncommon manifestation of NBD.

Observation:

  • A case of NBD presenting with severe gait instability and spasticity is described.
  • Spinal MRI revealed extensive, confluent T2 hyperintensities throughout the entire spinal cord with contrast enhancement.

Findings:

  • The patient experienced marked improvement following high-dose steroid therapy.
  • Follow-up MRI confirmed significant resolution of the myelitis.

Implications:

  • This report details the first description of longitudinal myelitis affecting the entire spinal cord in NBD.
  • NBD should be considered in the differential diagnosis of extensive longitudinal myelitis.
