Background:

  • Low uptake of HIV voluntary counseling and testing (VCT) hinders global HIV prevention efforts.
  • Only 10% of infected individuals are aware of their HIV status, limiting treatment access.
  • Home-based VCT models are explored to improve testing rates and prevent new infections.

Purpose of the Study:

  • Identify and appraise studies on home-based HIV VCT implementation in developing countries.
  • Determine if home-based HIV VCT improves testing outcomes compared to facility-based models.

Main Methods:

  • Searched MEDLINE, EMBASE, CENTRAL, AIDSearch, Cochrane Library, LILACS, CINAHL, and Sociofile (up to February 2007).
  • Included randomized controlled trials (RCTs) and non-randomized trials comparing home-based VCT with other models.
  • Extracted data and expressed findings as odds ratios (OR) and relative risks (RR) with 95% confidence intervals (CI).

Main Results:

  • Two studies (one cluster-RCT, one pre/post-cohort) were included, both from developing countries.
  • Home-based HIV VCT showed very high acceptability and uptake compared to facility-based testing.
  • Home-based testing increased VCT acceptance by 4.6 times (RR 4.6) and result delivery by 5.23 times (OR 5.23).

Conclusions:

  • Home-based HIV testing and results delivery increase testing uptake.
  • Limited evidence and study limitations necessitate further research before large-scale implementation.
  • Home-based models show promise for improving HIV testing rates in resource-limited settings.
