Summary

Primary retinectomy without scleral buckling effectively reattaches the retina in cases of anterior proliferative vitreoretinopathy. This surgical approach yields good anatomic success and improved visual acuity, with a low rate of complications.

Background:

  • Anterior proliferative vitreoretinopathy (APVR) complicates rhegmatogenous retinal detachment (RRD).
  • Scleral buckling is a common surgical adjunct, but its absence in primary retinectomy for APVR requires evaluation.

Purpose of the Study:

  • To assess the functional and anatomic outcomes of primary retinectomy without scleral buckling for APVR.
  • To evaluate eyes that did not undergo prior buckling surgery.

Main Methods:

  • Retrospective, nonrandomized, interventional case series.
  • Reviewed 123 consecutive cases of retinectomy for RRD with APVR.
  • Primary outcome: anatomic success (retinal reattachment); Secondary outcomes: visual acuity, reoperation rate, hypotony, complications.

Main Results:

  • Achieved a 77.2% primary anatomic success rate, with a 95.9% final attachment rate after reoperations.
  • Significant visual acuity improvement from 2.10 to 1.44 logMAR (P < .001).
  • Low incidence of postoperative hypotony (4.1%) and manageable complication rates.

Conclusions:

  • Primary retinectomy without scleral buckling demonstrates favorable anatomic and functional results for APVR.
  • This approach offers a viable alternative for managing complex retinal detachments.
