Summary
Recurrent pleomorphic adenoma in children is rare but linked to hypocellular subtypes and prior tumor enucleation. Total parotidectomy is recommended to prevent recurrence in pediatric parotid gland tumors.

- A case of a multicentric, doubly recurrent parotid pleomorphic adenoma in a 9-year-old child is presented.

- All resected nodules exhibited the hypocellular variant of pleomorphic adenoma.
Findings:
- Female sex, young age, hypocellular histological subtype, and tumor enucleation are identified as potential risk factors for recurrence.
- The presented case suggests a strong correlation between prior surgical enucleation, the hypocellular subtype, and tumor recurrence.
- Multifocal nature of the tumor may also contribute to recurrence.

- Tumor enucleation appears to be a significant risk factor for parotid pleomorphic adenoma recurrence in pediatric patients.
- Total parotidectomy is proposed as the treatment of choice for pleomorphic parotid adenoma, even in childhood, to minimize recurrence.
