Transcranial sonography differentiates subtypes of progressive supranuclear palsy (PSP). PSP-Parkinsonism (PSP-P) typically shows a hyperechogenic substantia nigra, while Richardson's syndrome (RS) exhibits a normoechogenic substantia nigra and enlarged third ventricle.

Background:

Transcranial sonography (TCS) reveals substantia nigra (SN) echogenicity differences between Parkinson's disease (hyperechogenic) and progressive supranuclear palsy (normoechogenic).
A new classification distinguishes Richardson's syndrome (RS) and PSP-Parkinsonism (PSP-P) within PSP.

Purpose of the Study:

To investigate if SN echogenicity and third ventricle dimensions on TCS can differentiate between RS and PSP-P.

Main Methods:

TCS was performed on 34 PSP patients (27 RS, 7 PSP-P).
Substantia nigra echogenicity and third ventricle size were analyzed and compared between RS and PSP-P groups.

Main Results:

Most PSP-P patients (6/7) showed a hyperechogenic SN.
The majority of RS patients (26/27) presented with a normoechogenic SN and an enlarged third ventricle (mean 10.3 mm).
PSP-P patients had a normal-sized third ventricle (mean 7.1 mm).

Conclusions:

TCS findings, specifically SN echogenicity and third ventricle size, can help differentiate between PSP-P and RS.
These sonographic differences may reflect underlying pathological distinctions between the two PSP subtypes.
