This case report details a rare instance of nonfunctional parathyroid carcinoma presenting as a large mediastinal mass. Despite imaging, the tumor was difficult to detect, highlighting diagnostic challenges in rare endocrine cancers.

Background:

  • Parathyroid carcinoma is a rare endocrine malignancy.
  • Nonfunctional variants present diagnostic challenges due to lack of hyperparathyroidism symptoms.

Observation:

  • A 65-year-old female presented with a large, painless superior mediastinal mass.
  • Imaging studies revealed a solid tumor, but Technetium (Tc-99) sestamibi scan showed a hypofunctioning area.
  • Preoperative diagnosis was uncertain, considering thyroid tumor or metastatic lymph node.

Findings:

  • Intraoperative frozen section suggested parathyroid carcinoma.
  • Histopathology confirmed parathyroid carcinoma with positive chromogranin A staining.
  • The tumor was negative for parathyroid hormone (PTH) immunostaining.

Implications:

  • This case underscores the possibility of nonfunctional parathyroid carcinoma presenting as a significant mediastinal mass.
  • Diagnostic modalities may fail to detect these rare tumors, necessitating high clinical suspicion.
  • Successful surgical resection and adjuvant radiotherapy led to a favorable long-term outcome without recurrence.
