Severe metal fume fever from galvanized steel exposure can cause serious lung damage. Early steroid treatment may significantly reduce illness severity and duration, as seen in two case studies.

- The 51-year-old patient, treated 3 days post-exposure, had a more protracted and severe course.
- HRCT confirmed diffuse alveolar damage in both individuals, indicating significant lung injury.

- This report highlights the potential for severe lung injury from MFF, contrary to its usual benign presentation.
- Prompt corticosteroid administration may be a crucial therapeutic intervention for severe MFF.
- Occupational safety measures, including adequate ventilation, are critical when working with galvanized steel.
