Area of Science:

  • Radiation oncology
  • Medical physics
  • Urologic oncology

Background:

  • External beam radiotherapy (EBRT) is a key treatment for prostate cancer (PCa).
  • Dose escalation beyond 74 Gy using 3D-CRT or IMRT improves therapeutic index.
  • Higher doses correlate with better biochemical disease-free survival but not local recurrence or overall survival.

Purpose of the Study:

  • To review recent advances in radiation therapy techniques for prostate cancer.
  • To explore the potential of dose escalation and hypofractionation.
  • To discuss novel technologies like VMAT, IGRT, and particle beam irradiation.

Main Methods:

  • Review of current literature on advanced radiation therapy techniques for PCa.
  • Analysis of dose escalation strategies and hypofractionation data.

  • Discussion of emerging technologies such as volumetric modulated arc therapy (VMAT), image-guided RT (IGRT), and carbon ion RT.
  • Main Results:

    • Dose escalation in EBRT for PCa improves biochemical outcomes.
    • Hypofractionation is a potential option, but requires further validation in Phase III trials.
    • Advanced RT technologies facilitate dose escalation and hypofractionation, with potential for focal dose escalation.

    Conclusions:

    • Advances in RT technology are crucial for optimizing prostate cancer treatment.
    • Future directions include hypofractionation, particle therapy, and focal dose escalation to enhance efficacy and minimize toxicity.
