A Retrospective Study on Endoscopic Surgery for the Treatment of Paravertebral Abscess in Spinal Tuberculosis Patients

Background:

  • Abdominal tuberculosis (TB) encompasses infections of the gastrointestinal tract, peritoneum, mesentery, lymph nodes, liver, spleen, and pancreas.
  • Adhesive peritonitis and nodal disease are the most prevalent forms of abdominal TB in pediatric populations.

Observation:

  • A retrospective study analyzed 13 pediatric cases of abdominal TB diagnosed between 1995 and 2008.
  • Common presentations included abdominal distension, pain, fever, and abdominal masses.
  • Diagnostic methods involved ascitic fluid analysis, imaging, histopathology, and microbiological tests.

Findings:

  • Peritoneal involvement was most common (9 cases), followed by abdominal lymph nodes (7 cases).
  • Ascitic fluid typically showed exudative, lymphocyte-predominant fluid.
  • Histopathological and microbiological confirmation was achieved in 5 and 3 patients, respectively.

Implications:

  • In high TB prevalence areas with limited diagnostic facilities, empirical treatment based on clinical suspicion is viable.
  • A positive response to antituberculous therapy can serve as indirect diagnostic confirmation.
  • Prompt management of abdominal TB is essential to prevent complications, though multifocal disease may lead to sequelae.
