Background:

  • Marjolin ulcer, malignant transformation in burn scars, has been documented since 1828.
  • Few reports exist on Marjolin ulcer incidence in Oriental populations.
  • Burn scar carcinoma predominantly affects extremities.

Observation:

  • A study at Chang Gung Memorial Hospital identified 11 burn scar carcinoma cases between 1989 and 2008.
  • The majority of cases were squamous cell carcinoma (SCC), with one verrucous carcinoma.
  • Most patients underwent surgical intervention, including excision, grafting, or amputation.

Findings:

  • Prognosis for advanced scar cancer is poor, with risks of recurrence and metastasis.
  • Six patients remained tumor-free post-treatment, while five were lost to follow-up.
  • Squamous cell carcinoma is the most common type of scar malignancy.

Implications:

  • Aggressive initial burn wound care, including early grafting and diligent scar management, is vital for prevention.
  • Preventing repeated ulceration in burn scars can reduce the incidence of malignancy.
  • Timely surgical intervention and regular patient follow-up are essential for managing burn scar carcinoma.
