Kidney Transplant II: Surgical Procedure01:26

Kidney Transplant II: Surgical Procedure

Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living donor...
Kidney Transplant I: Introduction01:28

Kidney Transplant I: Introduction

A kidney transplant is a surgical approach that involves replacing a non-functioning kidney with a healthy one from a donor. This procedure is often a treatment option for end-stage renal disease (ESRD) patients. The method requires careful recipient selection, including evaluating various medical and psychosocial factors. These criteria vary between transplant centers but generally include assessments of the patient's overall health, adherence to medical recommendations, and lifestyle...

Kidney Transplant III: Nursing Management

Postoperative Nursing Management for Kidney Transplant PatientsPostoperative nursing management care includes monitoring the surgical site, encouraging early movement, and promoting lung health through breathing exercises. Nurses also administer prescribed medications like H2-blockers, such as famotidine, or proton pump inhibitors, like omeprazole, to help prevent gastrointestinal ulcers and bleeding. Fungal infections in the mouth and bladder can result from immunosuppressive and antibiotic...

Pharmacokinetics in Pediatric Patients: Drug Excretion

In pediatric medicine, understanding the renal function and drug elimination nuances is crucial for administering safe and effective treatments. Newborns, in particular, display markedly slower renal functions than adults, profoundly affecting how drugs are cleared from their bodies. This slower drug clearance requires clinicians to extend the dosing intervals for many medications to prevent drug accumulation and toxicity while ensuring therapeutic efficacy. One key area where these adjustments...
Nephrons01:10

Nephrons

The kidneys are intricate organs with millions of working units known as nephrons. Each nephron features two major structures: the renal corpuscle, which facilitates blood plasma filtration, and the renal tubule, which handles the glomerular filtrate. Blood supply is directly linked to the nephrons. The renal corpuscle consists of the glomerulus, a capillary network, and the Bowman's capsule, a double-walled epithelial structure that encases the glomerulus. The filtering of blood plasma happens...

Acute Kidney Injury IV: Diagnostic Studies and Prevention

Accurate diagnosis and effective prevention are critical in managing Acute Kidney Injury (AKI), which is linked to high mortality rates ranging from 10% to 80%. Timely recognition of at-risk patients and careful monitoring can significantly reduce the likelihood of kidney damage.Diagnostic Assessments:The diagnostic process starts with a comprehensive medical history to identify prerenal, intrarenal, and postrenal causes.Prerenal causes, such as dehydration, hypotension, or blood loss, should...

Summary

Pediatric nephrectomies increased significantly, with more procedures for Wilms' tumor and multicystic dysplastic kidney (MCDK). Fewer nephrectomies were performed for pelvi-ureteric junction (PUJ) obstruction, possibly due to improved prenatal hydronephrosis management.

Area of Science:

  • Pediatric Surgery
  • Urology
  • Nephrology

Background:

  • Advances in preoperative diagnostics, surgical techniques, and postoperative care have refined pediatric surgical practices.
  • Improved understanding of congenital renal anomalies has led to evolving management strategies.

Purpose of the Study:

  • To analyze the indications for nephrectomy in pediatric patients.
  • To review trends in pediatric nephrectomy from 1996 to 2008 at a children's hospital in Tunis.

Main Methods:

  • A retrospective review of 80 pediatric nephrectomy cases.
  • Comparison of two time periods: 1996-2000 and 2001-2008.

Main Results:

  • The annual number of nephrectomies significantly increased over the study period.
  • Indications shifted, with a notable rise in nephrectomies for Wilms' tumor and multicystic dysplastic kidney (MCDK).
  • A significant decrease in nephrectomies for pelvi-ureteric junction (PUJ) obstruction was observed, while rates for vesico-ureteric reflux (VUR) remained stable.

Conclusions:

  • The overall rate of pediatric nephrectomies, including partial procedures, has risen.
  • Decreased nephrectomies for PUJ obstruction may reflect better management of prenatal hydronephrosis.
  • Increased proportions of nephrectomies for neoplastic diseases and MCDK highlight changing pediatric renal pathology trends.
