Summary

A new grading system for retinal pigment epithelium (RPE) tears after anti-VEGF therapy shows prognostic value. Lower grade tears indicate better visual outcomes, while Grade 4 tears suggest a poor prognosis.

Background:

  • Age-related macular degeneration (AMD) is a leading cause of vision loss.
  • Anti-VEGF therapies are standard treatments for exudative AMD.
  • Retinal pigment epithelium (RPE) tears can occur after anti-VEGF treatment.

Observation:

  • A retrospective review of 21 eyes with RPE tears post-anti-VEGF therapy was conducted.
  • A novel grading scale for RPE tears was developed based on greatest linear diameter and foveal involvement.
  • Tear grades were correlated with visual acuity, anatomical outcomes, and treatment response.

Findings:

  • RPE tears were graded 1-4 based on size and foveal involvement.
  • Lower grade tears (1-3) were associated with better visual acuity and response to anti-VEGF therapy.
