Background:

  • Spontaneous preterm delivery (SPD) is a leading cause of neonatal morbidity and mortality.
  • Understanding inherited predispositions to SPD is crucial for risk assessment and prevention strategies.

Purpose of the Study:

  • To assess the inherited predisposition to spontaneous preterm delivery by analyzing intergenerational delivery data.

Main Methods:

  • Retrospective cohort study of mother-daughter pairs from the Aberdeen Maternity Neonatal Databank.
  • Multilevel logistic regression analysis was used to compare SPD rates between exposed and unexposed cohorts.
  • Exposed cohort included women born preterm or whose mothers had a history of SPD.

Main Results:

  • Women born spontaneously preterm had significantly higher odds of delivering preterm babies (OR 1.49).
  • A history of SPD in mothers also increased the risk of SPD in daughters (OR 1.35).
  • Other risk factors identified include young maternal age, lower socioeconomic status, smoking, low BMI, and previous preterm delivery.

Conclusions:

  • Women born spontaneously preterm have an increased risk of spontaneous preterm delivery in their own pregnancies.
  • Familial patterns and inherited factors play a significant role in spontaneous preterm delivery.
