Drugs that Stabilize Microtubules01:15

Drugs that Stabilize Microtubules

Microtubules are dynamic structures that undergo cycles of catastrophe and rescue. The microtubules play a central role in cell division by forming the spindle apparatus for segregating the chromosomes. This makes them ideal targets for regulating dividing cells in tumors and malignant cancer cells. Microtubule stabilizing drugs help stabilize the microtubule formation and promote its polymerization. Paclitaxel was the first microtubule stabilizing agent used as anticancer drug in chemotherapy...
Treatment Resistant Cancers02:56

Treatment Resistant Cancers

Cancer is the second leading cause of death in the United States. A cancer cell is genetically unstable and hence can mutate faster. They can also modify their microenvironment and escape immune surveillance. The difficulties in treating cancer are further compounded by the emergence of rapid resistance to anticancer drugs. The most common ways to attain resistance in cancer cells include alteration in drug transport and metabolism, modification of drug target, elevated DNA damage response, or...

Summary

Mifamurtide, a liposomal muramyl tripeptide phosphatidyl ethanolamine (L-MTP-PE), activates immune cells to treat osteosarcoma lung metastases. It improves survival rates by 8-13% when combined with chemotherapy, with manageable side effects.

Area of Science:

  • Immunology
  • Pharmacology
  • Oncology

Background:

  • Mifamurtide (L-MTP-PE) is approved in Europe for osteosarcoma treatment.
  • It utilizes liposomes with synthetic phospholipids to activate macrophages.
  • The drug targets immune cells, not directly cytotoxic to tumor cells.

Purpose of the Study:

  • To evaluate the efficacy and safety of mifamurtide in osteosarcoma treatment.
  • To understand the mechanism of action of mifamurtide in activating immune responses.
  • To present an algorithm for managing mifamurtide's short-term toxicities.

Main Methods:

  • Mifamurtide was administered with chemotherapy to osteosarcoma patients.
  • Immune cell activation and survival rates were monitored.
  • Pre- and postmedication protocols, including ibuprofen, were used to manage side effects.

Main Results:

  • Mifamurtide improved 5- and 6-year overall survival by 13% and 8% respectively in metastatic and non-metastatic patients.
  • Short-term toxicities like fever and flu-like symptoms were reduced with premedication.
  • No long-term side effects have been reported to date.

Conclusions:

  • Mifamurtide demonstrates significant survival benefits in osteosarcoma patients.
  • Its mechanism involves immune activation rather than direct cytotoxicity.
  • Mifamurtide represents a successful collaboration in pediatric cancer treatment.
