Summary

This case report details a rare instance of endophthalmitis following non-penetrating deep sclerectomy for advanced open-angle glaucoma. Despite initial success, the patient experienced vision loss due to severe retinal necrosis.

Background:

  • Non-penetrating deep sclerectomy with Mitomycin C is a surgical option for advanced open-angle glaucoma.
  • Management of glaucoma in a patient's only functional eye presents unique challenges.

Observation:

  • A patient underwent successful non-penetrating deep sclerectomy, preserving visual acuity and reducing intraocular pressure.
  • Two months post-surgery, the patient developed symptoms of bleb-related endophthalmitis.

Findings:

  • Treatment for endophthalmitis, including pars plana vitrectomy, was unsuccessful in preventing severe retinal necrosis.
  • This resulted in significant loss of visual acuity.

Implications:

  • This is the first reported case of endophthalmitis occurring after non-penetrating deep sclerectomy.
  • Highlights a potential severe, sight-threatening complication of this glaucoma surgical procedure.
