For individuals with type 1 diabetes mellitus (T1DM), consuming isomaltulose before exercise leads to better blood glucose control and improved fat oxidation compared to dextrose.

- Eight T1DM participants consumed 75g of either dextrose (GI=96) or isomaltulose (GI=32) two hours before a 45-minute treadmill run at 80% VO2peak.
- Blood glucose (BG) levels were monitored for 2 hours pre-exercise and 3 hours post-exercise.
- Cardiorespiratory parameters, substrate utilization (CHO and lipid oxidation), and BG responses were analyzed using repeated-measures ANOVA.

- Isomaltulose ingestion resulted in a smaller pre-exercise blood glucose increase and a delayed peak compared to dextrose.
- During the final exercise minutes, isomaltulose led to lower carbohydrate oxidation and higher lipid oxidation rates compared to dextrose.
- Post-exercise, blood glucose levels remained significantly lower with isomaltulose for up to 180 minutes.
Conclusions:
- Isomaltulose consumption improves blood glucose management during and after exercise in individuals with T1DM.
- The benefits are attributed to reduced carbohydrate utilization and enhanced lipid oxidation during the later stages of exercise.
