Context:

Lithium therapy requires regular monitoring of lithium, creatinine, and thyroid-stimulating hormone (TSH) due to potential thyroid and kidney effects.
Dutch bipolar disorder guidelines from 2001 mandate biannual checks for these parameters.

Purpose:

To assess adherence to 2001 Dutch guidelines for monitoring lithium, creatinine, and TSH levels among psychiatrists and general practitioners (GPs).

Summary:

A 2004 study of a Dutch region found that 11% of psychiatrists and 25% of GPs did not check lithium levels biannually.
Monitoring of creatinine and TSH was also suboptimal, with rates of 65.8% and 68.3% for psychiatrists, and 61.2% and 58% for GPs, respectively.
GPs demonstrated significantly lower adherence to monitoring frequency compared to psychiatrists.

Impact:

The findings suggest a need for improved systems to ensure guideline adherence in lithium treatment monitoring.
Recommendations include implementing a laboratory-based warning system for missed checks and potentially restricting lithium prescription to psychiatrists.
