[Optimization of reconstructive surgery on the frontal sinus]
Abstract:
Results of the treatment of three patients with frontal sinus injuries of different severity demonstrate that it is desirable to start surgical intervention for their management in the early period after injury, i.e. as soon as the acute process is completed. The extent of surgery should be estimated based on the results of computed tomography of the accessory sinuses of the nose in the preoperative period. The frontal sinus was approached through the modified Zutter skin incision which allowed to avoid a cosmetic facial defect, an injury to temporal muscles and frontal branch of the facial nerve. The defect in the anterior wall was used to revise the frontal sinuses, remove small bone fragments and foreign bodies, and examine the frontonasal anastomosis. Large bone fragments were fixed with titanium miniplates. An extensive bone defect was covered with a titanium mesh. Good cosmetic and functional results were obtained in all the patients in the postoperative period.
