Summary

Nucleic acid amplification technology (NAT) testing significantly improved blood safety, reducing viral transmission risks. Discontinuing or shortening fresh frozen plasma (FFP) quarantine may offer logistical benefits with minimal impact on residual risk.

Background:

  • Mandatory nucleic acid amplification technology (NAT) testing for Hepatitis C virus (HCV) in Germany led to reduced fresh frozen plasma (FFP) quarantine periods.
  • Introduction of NAT for HIV-1 and anti-HBc testing further decreased transfusion-transmitted viral risks.

Purpose of the Study:

  • To evaluate the impact of NAT testing on viral blood safety in Germany.
  • To assess the feasibility of discontinuing or reducing FFP quarantine periods based on residual viral transmission risks.

Main Methods:

  • Analysis of over 40 million donations tested by HCV NAT.
  • Review of residual risk factors including minimal infectious dose, viral replication kinetics, NAT detection limits, and plasma volume transfused.

Main Results:

  • HCV NAT testing reduced residual risk by a factor of 10, with only one documented HCV transmission over 10 years.
  • Similar safety improvements were observed for HIV-1.
  • The remaining viral transmission risk is considered marginal.

Conclusions:

  • Discontinuing FFP quarantine is likely to increase residual risk only marginally.
  • Reducing the quarantine period to 4 weeks could enhance the viral safety of quarantined FFP and improve logistics.
