Background:

  • Diabetic autonomic bladder dysfunction is a known complication of diabetes mellitus.
  • It is generally considered rare in the pediatric population.
  • Type 1 diabetes mellitus (T1DM) requires long-term management and monitoring for complications.

Observation:

  • A 16-year-old female with a 10-year history of T1DM presented with symptoms suggestive of bladder dysfunction.
  • Urinary tract ultrasonography revealed a distended bladder with normal renal parenchyma.
  • Laboratory tests showed normal kidney function but elevated microalbuminuria.

Findings:

  • The patient was diagnosed with diabetic cystopathy.
  • Urodynamic studies indicated impaired bladder sensation, increased bladder capacity, and detrusor areflexia.
  • These findings are consistent with autonomic neuropathy affecting bladder function.

Implications:

  • Autonomic bladder dysfunction, specifically diabetic cystopathy, should be considered in the differential diagnosis of urinary issues in adolescents with T1DM.
  • Early recognition and management may help prevent progression and improve quality of life.
  • This case underscores the need for comprehensive screening of diabetic complications in pediatric patients.
