Vasculogenic erectile dysfunction and metabolic syndrome

Erectile dysfunction (ED) is common in older men and linked to systemic diseases. Penile Doppler ultrasonography can help evaluate vascular issues in patients with metabolic syndrome (MS).

Background:
- Erectile dysfunction (ED) affects over half of men aged 40-70.
- ED is characterized by the inability to achieve or maintain an erection sufficient for intercourse.
Purpose of the Study:
- To identify factors influencing penile vascular blood flow in ED patients.
- To assess the predictability of vascular flow in men with ED.
Main Methods:
- 163 male patients with ED completed the International Index of Erectile Function (IIEF) questionnaire.
- Blood tests measured cholesterol, triglycerides, and fasting blood glucose (FBG); Body Mass Index (BMI) was calculated.
- Penile color Doppler ultrasonography (PDU) evaluated penile blood flow patterns.
Main Results:
- No significant correlation was found between arterial insufficiency and metabolic syndrome (MS).
- A significant correlation was observed between veno-occlusive dysfunction and MS.
- Penile vascular pathologies were assessed in relation to hypertension, obesity, FBG, and cholesterol.
Conclusions:
- ED prevalence rises with age and systemic diseases.
- Standard evaluations may be insufficient for comprehensive ED assessment.
- Penile Doppler ultrasonography is recommended for evaluating penile vascular structures in ED patients with MS.
