Summary

A new Yale formula (YF) improves prostate cancer lymph node (LN) risk prediction. The YF better identifies patients with high-risk regional lymph node involvement compared to existing Roach and Nguyen formulas.

  • Derived the Yale formula (YF) from a subset (n = 1,460) and validated it on the remaining subset (n = 1,470).
  • Main Results:

    • The Yale formula (YF) demonstrated superior accuracy in predicting lymph node positivity (%LN+).
    • YF showed increased sensitivity (39.0% vs. 13.6%) in identifying high-risk patients (>15% %LN+ risk) compared to NF, with comparable specificity (94.9% vs. 98.8%).
    • The NF was overly restrictive, classifying only 2% of patients as high-risk.

    Conclusions:

    • The Yale formula (YF) outperforms both the Roach formula (RF) and Nguyen formula (NF) in predicting lymph node involvement risk.
    • YF is more effective in differentiating prostate cancer patients at high risk for lymph node metastasis.
