Hand-assisted retroperitoneoscopic live donor nephrectomy (HARP-LDN) offers a safe and effective alternative for kidney donation. This technique may reduce the learning curve associated with retroperitoneoscopic live donor nephrectomy.

Background:

  • Retroperitoneoscopic (RP) live donor nephrectomy (LDN) offers donor advantages but has a steep learning curve.
  • Hand-assistance (HA) may mitigate the challenges of the RP approach.

Purpose of the Study:

  • To present the initial experience with a novel hand-assist approach to retroperitoneoscopic live donor nephrectomy (HARP-LDN).

Main Methods:

  • A consecutive series of 10 patients underwent left HARP-LDN over one year.
  • The technique involved a 7 cm muscle-sparing incision for a hand-port and two endoscopic ports.

Main Results:

  • Mean operative time was 155 minutes with no open conversions.
  • Mean blood loss was 68 mL, and warm ischemia time was 2.5 minutes.
  • Average hospital stay was 2.7 days with minimal complications.

Conclusions:

  • The modified HARP approach for left LDN is safe, effective, and expeditious.
  • Larger studies are needed to confirm the benefits of this hand-assisted retroperitoneal technique.
