Summary

Hand-assisted retroperitoneoscopic live donor nephrectomy (HARP-LDN) offers a safe and effective alternative for kidney donation. This technique may reduce the learning curve associated with retroperitoneoscopic live donor nephrectomy.

Background:

Retroperitoneoscopic (RP) live donor nephrectomy (LDN) offers donor advantages but has a steep learning curve.
Hand-assistance (HA) may mitigate the challenges of the RP approach.

Purpose of the Study:

To present the initial experience with a novel hand-assist approach to retroperitoneoscopic live donor nephrectomy (HARP-LDN).

Main Methods:

A consecutive series of 10 patients underwent left HARP-LDN over one year.
The technique involved a 7 cm muscle-sparing incision for a hand-port and two endoscopic ports.

Main Results:

Mean operative time was 155 minutes with no open conversions.
Mean blood loss was 68 mL, and warm ischemia time was 2.5 minutes.
Average hospital stay was 2.7 days with minimal complications.

Conclusions:

The modified HARP approach for left LDN is safe, effective, and expeditious.
Larger studies are needed to confirm the benefits of this hand-assisted retroperitoneal technique.
