Summary

The reverse Less Invasive Stabilization System (LISS) effectively treated elderly patients with osteoporotic intertrochanteric hip fractures, achieving uneventful healing in all cases. This surgical approach shows promise for complex fractures in this vulnerable population.

Area of Science:

  • Orthopedic Surgery
  • Traumatology
  • Geriatric Medicine

Background:

  • Intertrochanteric hip fractures are common in the elderly.
  • Osteoporosis complicates internal fixation, leading to poor outcomes.
  • The Less Invasive Stabilization System (LISS) was adapted for reverse positioning.

Purpose of the Study:

  • To evaluate the efficacy of reverse LISS in treating elderly patients with osteoporotic intertrochanteric hip fractures.
  • To assess functional outcomes and complication rates.

Main Methods:

  • Retrospective review of 28 elderly patients (mean age 82.3 years) with intertrochanteric hip fractures.
  • All fractures treated with reverse LISS plating.
  • Evaluation of radiographic and clinical outcomes, including complications.

Main Results:

  • Mean blood loss: 92.4 mL; mean hospital stay: 8.7 days.
  • One minor wound hematoma; no radiographic signs of fixation failure (collapse, cutout, penetration).
  • 100% uneventful fracture healing with union by six months in all surviving patients.

Conclusions:

  • Reverse LISS plating is a viable option for intertrochanteric hip fractures in elderly patients with osteoporosis.
  • The technique facilitates event-free fracture healing.
  • Demonstrates successful management of complex fractures in a challenging patient group.
