Summary

Preoperative embolization of meningioma using lipiodol demonstrated successful tumor resection in most cases. However, delayed surgery increased complication risks, highlighting the importance of timing in this neurosurgical procedure.

Area of Science:

  • Neurosurgery
  • Interventional Radiology
  • Oncology

Background:

  • Meningiomas are primary tumors of the brain and spinal cord.
  • Preoperative embolization is a technique used to reduce tumor vascularity before surgical resection.
  • Lipiodol has been explored as an embolic agent in neurosurgical procedures.

Purpose of the Study:

  • To evaluate the efficacy and safety of preoperative embolization with lipiodol for meningioma treatment.
  • To assess the impact of the time interval between embolization and surgery on outcomes.
  • To analyze postoperative complications associated with lipiodol embolization.

Main Methods:

  • Retrospective analysis of four patients with meningioma undergoing preoperative embolization.
  • Superselective catheterization of tumor feeders via the external carotid artery.
  • Slow infusion of lipiodol followed by proximal coil occlusion.

Main Results:

  • Three out of four meningiomas were successfully resected 5 days post-embolization.
  • One meningioma resected 13 days after embolization required blood transfusion.
  • Two patients experienced postoperative complications: temporal muscle ischemia and transient edema.

Conclusions:

  • Preoperative embolization with lipiodol can be effective for meningioma treatment.
  • Shorter intervals between embolization and surgery appear to correlate with better outcomes.
  • Lipiodol's safety and ischemic effects as an embolic material warrant further investigation.
