Summary

The interlocking detachable coil (IDC) is a viable endovascular treatment for cerebral aneurysms, offering a useful alternative for complex cases. This study shows IDC embolization is possible and effective, even for ruptured aneurysms.

Background:

  • The Guglielmi detachable coil (GDC) was not available in Japan until February 1997.
  • The interlocking detachable coil (IDC) was the sole detachable and retractable coil option during the study period.

Purpose of the Study:

  • To evaluate the efficacy and safety of endovascular treatment for cerebral aneurysms using the interlocking detachable coil (IDC).

Main Methods:

  • Retrospective analysis of 25 cerebral aneurysm cases treated with IDCs between October 1993 and February 1997.
  • Treatments included intraaneurysmal coil embolization and parent artery or proximal occlusion.

Main Results:

  • 15 cases underwent intraaneurysmal coil embolization, achieving complete occlusion in 9, subtotal in 4, and partial in 2.
  • 9 cases were treated with parent artery or proximal occlusion.
  • One case of coil compaction and three instances of distal emboli (one symptomatic) were observed. Intraoperative bleeding occurred in one case.

Conclusions:

  • Endovascular treatment using IDCs is a feasible and valuable alternative for cerebral aneurysms, particularly those that are surgically challenging.
  • Treatment of poor-grade ruptured aneurysms remains difficult but is possible with IDC therapy.
