Summary

Combined treatment including radiosurgery offers a promising cure for cerebral arteriovenous malformations (AVMs). This approach, particularly for complex cases, shows reduced neurological risk and high obliteration rates.

Background:

  • Cerebral arteriovenous malformations (AVMs) pose significant treatment challenges.
  • Traditional treatment options for AVMs have limitations, especially for complex or giant lesions.

Purpose of the Study:

  • To evaluate the efficacy and safety of combined endovascular/surgical treatment followed by Gamma Knife radiosurgery for cerebral AVMs.
  • To assess the impact of adjunctive radiosurgery on treatment outcomes for previously untreatable AVMs.

Main Methods:

  • Retrospective analysis of 45 patients treated with combined therapy (embolization and/or surgery followed by radiosurgery) between 1992 and the study period.
  • AVMs were classified using Spetzler's Grading System.
  • Gamma Knife stereotactic radiosurgery was utilized as an adjunctive treatment modality.

Main Results:

  • Fifteen complete AVM obliterations were achieved in a mean follow-up of 16 months.
  • Two patients had residual AVMs after two years.
  • Morbidity was 8.8%, with no mortality.
  • Successful treatment of complex and giant AVMs in eloquent areas was observed.

Conclusions:

  • Combined endovascular/surgical treatment with subsequent radiosurgery is an effective strategy for managing cerebral AVMs.
  • This multimodal approach enhances the possibility of total cure, particularly for partially embolized AVMs and complex lesions.
  • Adjunctive radiosurgery can be performed with a low risk of neurological deficit, expanding treatment options for challenging AVM cases.
