Background:

Chronic illnesses often require complex medication regimens.
Effective management of chronic heart failure (CHF) is crucial to prevent acute exacerbations and reduce healthcare utilization.

Purpose of the Study:

To evaluate the role of pharmacists in managing patients with chronic heart failure.
To determine the impact of pharmacist interventions on healthcare costs and patient outcomes.

Main Methods:

Review of recent evidence on pharmacist involvement in CHF care.
Analysis of pharmacist interventions such as patient self-care education and collaborative team participation.

Main Results:

Pharmacist-led interventions, including self-care guidance and collaborative care, are associated with reduced urgent healthcare needs for CHF patients.
These interventions correlate with significant decreases in healthcare expenditures.

Conclusions:
